Download and print the application form which is in a MS-Word format, on A4 size good quality paper.

Paste one passport size photograph with glue at the space provided on the application form, attach two additional passport size photographs (do not staple) and complete all other parts of the application form.

Mail the completed application form enclosing the DD / Cheque of Rs. 1500 / Rs. 750 as applicable, Caste or Disability certificates to the Admissions Office, Indian Institute of Management Lucknow, Prabandh Nagar, IIM Road, Off Sitapur Road, Lucknow - 226013. It is recommended that the completed Application Form with enclosure is sent by Registered Post / SpeedPost so as to reach the Admissions Office, IIM Lucknow before closing time on the last date for receiving the application form.
